The owner or finder of said pet must fill out the following form in order for us to consider taking said pet. In the intake form below, we will ask things such as the cat's behavior, personality, bite history if any, veterinary records if any, medical history, what the cat is good with such as children or other pets, reason for surrender, as well as a required donation upon intake if no medical has been completed. Please allow 24–72 hours for a response to your application as we are busy saving lives. Thank you.
