Policy On Owner Surrender

The owner/finder of said pet must fill out the following form in order for us to consider taking this said pet. In the intake form below, we will ask things such as the cat's behavior, personality, bite history if any, vet records if any, medical history, what the cat is good with (kids, other pets, etc.), reason for surrender, as well as a required donation upon intake if no medical has been completed. Please give us 24 to 72 hours to respond to your application as we are busy saving lives. Thank you.
